This Personal Data Protection Application Form has been prepared to enable data subjects to exercise their rights under Article 11 of the Law No. 6698 on the Protection of Personal Data ("KVKK").

2. Application Topics

Data subjects may submit applications regarding the following matters in accordance with Article 11 of the KVKK (Law on Protection of Personal Data):

• To find out whether your personal data is being processed.
• If it has been processed, request information about it.
• To learn the purpose of processing and whether it is being used for its intended purpose.
• Knowing the third parties to whom it is transferred, whether domestically or internationally.
• Requesting correction of incomplete or incorrectly processed data.
• Requesting the deletion, destruction, or anonymization of personal data.
• Claiming compensation for damages incurred due to the unlawful processing of the transaction.

3. Application Methods

Requests within the scope of the Personal Data Protection Law (KVKK) may be submitted using one of the following methods:

4. Application Evaluation

Applications will be finalized within a maximum of 30 (thirty) days from the date the application is received by the data controller, in accordance with the Personal Data Protection Law (KVKK) and related legislation.

If the application also requires a cost, a fee may be charged according to the tariff determined by the Personal Data Protection Board.

5. Entry into force

This GDPR Application Form enters into force on the date of its publication on the yesido.com.tr website.
